APPLICATION FOR DOD PERSONNEL COUNTRY CLEARANCE REQUEST FOR ITALY

PERSONNEL COUNTRY CLEARANCE OVERVIEW

Important notice: The Office of Defense Cooperation (ODC) has begun accepting country clearance requests via the Department of Defense's global Aircraft and Personnel and Automated Clearance System (APACS). The APACS site can be accessed at the following Internet link:

While ODC Rome will continue to process visit requests submitted via traditional means; visitors are encouraged to migrate to APACS for all future visits. Personnel not using APACS should review the procedures listed below.

1. Personnel country clearance procedures for Italy are illustrated in detail in the DoD Foreign Clearance Guide (DoD 4500-54-G), Europe/Italy Section III. All DOD travelers, on Official Business, must coordinate THEATER Clearance through their appropriate MACOM in Europe. Personnel country clearance is required for the following categories of visits:

  • All visits to Italian government, Italian military facilities/installations and Italian industry plants.
  • Visits by Distinguished Visitors, such as Flag Officers and civilian equivalents.
  • Visits to the American Embassy Rome or an American Consulate in Italy.
  • Unit deployments in conjunction with exercises or training activity conducted within Italian Sovereign Territory.
  • Site Surveys.

2. Personnel Lead Time Requirements:

  • Ninety days for Site Surveys and/or Deployments in conjunction with Training or Exercises conducted within Italian Sovereign Territory.
  • Forty days for visits to Italian government agencies, military installations/facilities and Italian industries.
  • Ten days for visits to the American Embassy Rome or U.S. Consulates in Italy.
  • Five days for visits by two-star military officers and above, civilian equivalents and other distinguished visitors (applies only to visits to the following locations: U.S. Embassy Rome, U.S. Consulates and visits to U.S. Military Installations if no contact with Italian Officials is anticipated).

3. All DOD sponsored travelers must complete Level One Antiterrorism/Force Protection Training prior to departure from home station.

VISITS OF NON-NATO PERSONNEL

The Government of Italy (GOI) reserves approval authority for all visits to Italy by non-NATO personnel. U.S. personnel will not extend invitations to non-NATO personnel to visit locations in Italy without prior authorization by the GOI. This includes visits to military bases in Italy with primarily U.S. presence, which the GOI views as Italian bases and has assigned an Italian Base Commander. The ODC is the sole contact point for non-NATO visits to Italy. The DoD Foreign Clearance Guide contains the procedures to request visit authorization for non-NATO personnel.

NOTE: When visits are approved by the GOI, it is the responsibility of the visiting country to ensure compliance with immigration agreements for entry into Italy, including passport and visa requirements.

TRAINING FLIGHT AND HAZMAT CLEARANCES

1. The ODC is responsible for Diplomatic Flight Clearances for Exercises and Training activities conducted within Italian Sovereign Territory including: Transfer Flights, Training Flights and clearances for Hazardous Cargo transported in conjunction with those training activities. 2. The DAO is responsible for all other Diplomatic Flight Clearances for transfer and over-flight to include Hazardous Cargo clearances for Italy not associated with Exercises and Training conducted within Italian Sovereign Territory. The DAO phone number is (+39) 06-4674-2339.

INSTRUCTIONS FOR PERSONNEL COUNTRY CLEARANCE REQUEST FORM

PRIVACY ACT: The information contained in this form is subject to the Privacy Act Statement of 1974. Providing this information for the purpose of obtaining a Country Clearance/Visit Approval constitutes authorization for its release to appropriate offices for the purpose of facilitating such approvals.

1. This form cannot be used for Non-NATO Visit Requests. Refer to the DOD Foreign Clearance Guide for appropriate format and lead-time requirements.

2. Form must be filled out completely or will not be accepted.

3. A separate request must be submitted for each location to be visited.

4. A separate request must be submitted for visitors with different arrival and departure dates. (e.g. 5 visitors want to visit Camp Darby, Italy: 2 arrive on 1 December and depart on 5 December; the other 3 arrive on 2 December and depart on 6 December. Two separate requests must be submitted).

 

SECTION 1 – REQUESTING AGENCY INFORMATION

Block 1A: Name of Requesting Agency.

Block 1B: Name of the POC at the Requesting Agency responsible for follow-up on the status of the Request.

Block 1C: Phone Number of Individual Listed in Block 1B.

Block 1D: Unclassified E-mail Address of Individual listed in Block 1B (.mil or .gov ONLY).

Block 1E: Complete address of Requesting Agency.

 

SECTION 2 - VISIT INFORMATION

Block 2A: Provide Arrival Date (DD MMM YR) and Departure Date (DD MMM YR) to and from Italy.

Block 2B: Provide complete Name and Address of the location to be visited along with exact dates of the Official Portion of your visit in the DD MMM YR to DD MMM YR format (do not include travel time to and from Italy or any leave to be taken in conjunction with this trip).

Block 2C: Check appropriate box indicating if contact with Italian Military, Government or Industry personnel in an Official capacity is anticipated.

Block 2D: Provide name and rank (if applicable) of POC at the visit location.

Block 2E: Provide phone number of individual listed in Block 2D.

Block 2F: Provide name and rank (if applicable) of Italian POC for your visit (if meeting with Italian Officials).

Block 2G: Provide phone number of individual listed in Block 2F (if applicable).

Block 2H: Provide highest classification level of material to be discussed during your visit.

Block 2I: Check appropriate box indicating if your visit is at the invitation of Italy (that individual should be your Italian POC listed in Block 2F) or if your visit is initiated by the U.S. NOTE: An invitation to visit by an in-country U.S. industry representative is not considered to be initiated by the country of Italy. Additionally, that In-Country U.S. Industry Representative CANNOT be your Italian POC at the site to be visited.

Block 2J: Provide detailed purpose of the visit.

 

SECTION 3 - INDUSTRY VISIT ADDITIONAL INFORMATION:

Block 3A: FOR INDUSTRY VISITS ONLY: Check appropriate box indicating what your visit is pertinent to.

Block 3B: FOR INDUSTRY VISITS ONLY: Specify Equipment, Weapons System, FMS Case, etc as per box checked in Block 3A.

 

SECTION 4 - VISITOR INFORMATION

FOR EACH VISITOR PROVIDE (continuation sheets may be used):

NAME : (last, first, MI).

RANK/GRADE : Provide rank and pay grade (if applicable).

COMPANY : Provide Requesting Agency Name. If visitor is a contractor working for DOD provide his/her Company Name (i.e. Raytheon, BAE, etc).

TITLE : Provide the visitor’s title.

SSN : Provide the visitor’s Social Security Number.

SECURITY CLEARANCE : Indicate current Security Clearance held.

AT/FP TNG DATE : Indicate date AT/FP was last completed or “will be completed prior to arrival”.

DATE OF BIRTH : Visitor’s Date of Birth in DD MMM YR format.

PLACE OF BIRTH : Visitor’s Birth Place.

NATIONALITY : Indicate the visitor’s nationality.

PASSPORT NUMBER : Provide the visitor’s passport number. PENDING, MIL ID or DOD ID are acceptable.
